there are many other influential factors as well one also should also keep in mind. key factors if given it is hard not to come out with a success
it is important to 1. identify where the organization is today by faciliatiing self-assessment by key players in the entity.
2. with the key players agree what the new ideal organization should look like
define new vision and objectives
3. put in place the plan, timelines, metrics and responsibilities to achive successful
progression to the goals

Your explanations are O.K. but just theoretical part of the issue.In practice, there are many details to be paid attention on the way to successful change.For example, harmony in group,benefiting from the conflicts in team,having a real leader and some potential ones,movitating team members to reach a high level of total quality in organization,making sure that objectives and missions are very clear and understadable for everybody in organization,giving the idea to everbody that individual success and team success aren't contrast things but complete each other.
    if you take into consider those factors while establishing the theoratical base for success,you most probably achieve your goals(not definitely because,we shouldn't forget the role of external factors.
